In women it can be PCOS, hormone imbalances, thyroid problems, endometriosis, menopause, fallopian tube blockages, a misshaped uterus, fibroids, pelvic scar tissue, etc, etc, etc
In males it can be due to poor sperm quality, Klinefelter’s syndrome, prior infection (mumps, chlamydia, gonorrhea, etc), testosterone deficiency, anti-sperm antibodies, deformities, etc, etc, etc.
In everyone, stress, obesity, drugs, environmental factors, etc can also play a role.

For a woman: blocked or scarred fallopian tubes
